Before you are scheduled for a surgery appointment with us, a consultation is necessary.
Consultations are approximately 15 minute appointments for the doctor to determine your best course of treatment. Consultations are usually done in the afternoons. Please call us at (281) 469-2873 in order to schedule.
After you’ve set your appointment date with our office, you will be required to fill out an intake form if you are a new patient. For your convenience, you can fill out the intake form below or in-person at the office.

Medicare doesn’t cover most dental care (including procedures and supplies like cleanings, fillings, tooth extractions, dentures, dental plates, or other dental devices). Medicare Part A (Hospital Insurance) will pay for certain dental services that you get when you’re in a hospital.
